You will complete Block 33 with the information provided.

3. Exercise 14-2:

You will complete the Mary S. Patient Medicare Primary CMS-1500 claim (Exercise 14-2 on page 530). Download the PDF claim form above (you won't need to use the claim form in the Appendix as mentioned in #1 of exercise) and save it as "YourNameMarySPatient" when completed. This exercise has specific instructions, so please follow them carefully. Compare your claim with the completed claim in Figure 14-8 on page 531. Your claim should look like that before you submit it. This is a great low-stress opportunity to successfully complete a Medicare claim form!
